To help referring clinicians prepare the necessary documentation and ensure timely, appropriate triage, we have created this Referral Pack. The forms and guidance included support our collaborative model of care and ensure our clinical resources are directed where they’re needed most.
This pack includes:
- Referral Form
- General and Physical Seating Assessment Forms
- Referral Priority Scoring Scale
- Risk Assessment Form
To help ensure a smooth referral process, we ask that the following steps are followed
- Referral Form – Page 1
This form must be fully completed, including:
- Referring clinician’s contact details
- Signature from the OT manager or clinical coordinator
Why OT Manager Sign-off Is Requested
- Ensures the OT manager is aware of and engaged in the process from the start
- Helps identify the most appropriate referral pathway
- Supports communication in cases where teams change or rotate

We’ve introduced a new scoring tool to help us triage referrals based on urgency.
- Referring clinicians should complete Page 3 of the referral pack.
- The tool is designed to be intuitive, but we welcome any queries.

These are important for all referrals unless the person was recently seen (within ~6 months).
