Feedback Form

We welcome any feedback that will help us improve our service and your experience. Read our Comments, Compliments, Concerns and Complaints Policy.

We take all feedback (including comments, compliments, concerns and complaints) seriously. This provides us with opportunities to improve and maintain the high standards we strive to achieve across our disability services, shops and fundraising activities. We recognise that sometimes we get things wrong. We welcome any feedback that will help us improve our service and your experience.

We support the HSE’s Your Service, Your Say initiative. You can read Enable Ireland's Comments, Compliments, Concerns and Complaints Policy here.
Please complete our Feedback Form below to provide us with details of your comment, compliment, concern or complaint.

How can I provide feedback?

Option 1: Talk to a member of staff (first point of contact) who will be able to manage your feedback. Ideally feedback should first be raised directly with the staff member. This is the preferred option as it may help achieve a resolution to a complaint as early as possible. You may also ask to speak to a local service manager or retail manager depending on the issue.

Option 2: Complete the Enable Ireland Feedback Form available onsite in our reception or shop. Completed forms can be left with the reception area of your local service or retail outlet. You may also give it to a member of staff. This option can be used in the event that the complaint cannot be raised through a first point of contact.

Alternatively, you may prefer to complete the online version of the Feedback Form available below.
