Rialto Hub Service works in partnership with those who use our services to achieve maximum independence, choice and inclusion in their local/wider communities.    
 
In the heart of Rialto and within easy reach of the Red Luas line and public bus transport service, Rialto Hub Service primarily provides services to adults with physical disabilities. Using the Person Centred Planning (PCP) approach in line with the HSE guidelines for “New Directions”, enables adults with physical disabilities, to be in control of their service, to make informed choices about how they want to live their life, now and in the future with support from Staff who have the necessary skills and training. We do this by listening, learning and adapting our service accordingly to meet each person’s expressed needs and wishes.

Rialto Hub Service values person-centeredness, recognising the uniqueness of every individual, whilst we encompasses respect, self-determination, empowerment, and understanding. Person-centred planning enables a person to identify their dreams and goals, and what is required to make those possible. We will respect the right of each individual to make their own decisions, and we will provide support to facilitate. including access to advocacy services.
